The Health and Housing Practitioner Checklist is based upon the Department of Health Assessment Tool, and has been developed for use by those who come into contact with vulnerable householders. Home Safety Action Form
To pilot the Home Safety Action Form, Mendip Care & Repair has joined forces with Somerset Fire & Rescue Service, Avon & Somerset Constabulary Crime Prevention Team & Bobby Scheme, the PCT Falls and Accident Prevention Team, Care Direct to pilot a "first across the threshold" scheme, whereby front-line staff and officers complete a checklist to identify people who could benefit from referral to one of the other agencies in the Group.
